Job summary

Chapters Health System is seeking an Admission RN to assess patients in their residence or hospital and initiate care into our services. You will serve as an ambassador and coordinate with Care Navigation to ensure smooth transitions.

The role requires an active RN license, at least 1 year of nursing experience (hospice/hospital preferred), and proficiency with EMR/EHR systems. Valid driver’s license and reliable transportation are needed; English fluency is essential, Spanish preferred.

Qualifications

• RN license in the state where employed is required.
• Minimum of one year nursing experience; hospice or hospital experience preferred.
• Experience with EMR/EHR systems is required.
• Strong clinical assessment skills; background in emergency/ICU is preferred.
• Must be able to communicate effectively in English; bilingual in Spanish preferred.

Responsibilities

• Complete clinical admissions for all Company services.
• Explain services to patients and families empathetically; address questions and concerns.
• Educate patient/family on admission requirements and immediate needs.
• Complete informed consents and initial assessments.
• Initiate Plan of Care with physicians to address needs.
• Perform skilled nursing interventions to prevent complications and maximize comfort.
• Obtain physician orders and ensure timely documentation.
• Represent the Company at facilities and coordinate referrals; liaise with care teams.
• Use SBAR for clinical communication; coordinate with Care Navigation for follow-up.
• Assist with safe discharge planning and ensure timely transitions.
