Job summary

Chapters Health System in Florida seeks an Admission RN to assess patients in their residence or hospital and initiate care with our services. You’ll act as a liaison to Care Navigation and represent the organization with empathy and professionalism.

Ideal candidates hold an RN license in the state, have at least 1 year of nursing experience (hospice/hospital preferred), and familiarity with EMR/EHR systems. BLS certification required; valid driver’s license a must.

Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience (hospice or hospital preferred).
• Experience with EMR/EHR systems.

Responsibilities

• Completes clinical admissions for all Chapters Health services, explains services empathetically.
• Identifies admission barriers and addresses concerns.
• Educates patient/family on admission requirements and immediate needs.
• Obtains physician orders and initiates Plan of Care.
