Job summary

Chapters Health System seeks an Admission RN to assess patients in their place of residence or in hospitals and initiate care into the organization’s services. The role also serves as an ambassador of Chapters Health System, ensuring a seamless transition to Care Navigation.

The RN will utilize EMR/EHR systems, communicate effectively in English, and work in a team to manage multiple priorities while delivering compassionate, high-quality patient care.

Qualifications

• Current RN license in the state of work.
• Active BLS for healthcare professionals from AHA or Red Cross.
• Minimum 1 year RN experience; hospice or hospital preferred.
• Strong clinical assessment; EMR/EHR experience.
• English proficiency; bilingual in Spanish is a plus.
• Valid driver’s license and reliable transportation.
• Ability to work in a team and handle multiple priorities.

Responsibilities

• Explain services to patients and families with empathy.
• Identify admission barriers and provide responses.
• Educate on admission requirements and initial assessments.
• Coordinate physician orders and care plans.
• Document per policy and ensure timely referrals.
• Discharge planning and liaison with Care Navigation.
