Job summary

Chapters Health System is seeking an Admission RN to assess patients at their residence or in hospitals and initiate care within our service line. This role serves as an ambassador of the organization and ensures a seamless transition to Care Navigation.

The position requires an active RN license in Florida, with at least one year of nursing experience. Experience with EMR/EHR systems and strong communication skills (Spanish helpful) are preferred.

Qualifications

• Current RN license in the state where employed.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED/ICU background preferred.
• Ability to communicate in English; Spanish bilingual preferred.

Responsibilities

• Complete clinical admissions for all Chapters Health System services.
• Explain services to patients/families and address questions.
• Educate patients on admission requirements and immediate needs.
• Obtain consents and initial assessment according to policy.
• Coordinate care plans and communicate with physicians and Care Navigation.
• Support referrals and liaise with hospitals, patients, families, and facilities.
• Maintain SBAR communications with Care Navigation and after-hours staff.
• Ensure safe discharge and accurate documentation.
