Job summary

Chapters Health System in Florida is seeking an Admission RN to assess patients in their residence or hospital settings and initiate care within our services. The role emphasizes seamless transition to Care Navigation and acting as an ambassador for the organization.

Requirements include active RN license in the state, at least one year of nursing experience (hospice/hospital preferred), and EMR/EHR proficiency.

Qualifications

• Current RN license in the state where employed.
• Minimum of one year nursing experience; hospice or hospital preferred.
• Experience with EMR/EHR systems.
• Strong clinical assessment skills; ED or ICU background preferred.
• Effective English communication; bilingual in Spanish preferred.
• Professional presentation of services.
• Valid driver's license and reliable transportation.
• Ability to work in a team and manage multiple priorities.

Responsibilities

• Completes clinical admissions for all services and explains offerings empathetically.
• Educates patients/families on admission requirements and initial assessments.
• Initiates Plan of Care and communicates with physicians for immediate needs.
• Performs skilled nursing interventions to alleviate symptoms and comfort patients.
• Obtains physician orders and conducts documentation per policy.
• Represents the company at facilities and coordinates referrals across service lines.
• Maintains SBAR-based clinical communication with Care Navigation.
• Ensures safe discharge planning and after-hours coordination as needed.
