Job summary

Chapters Health System in Florida is seeking an Admission RN to assess patients at residence or in hospitals and initiate care within our care navigation services. The role represents the organization and ensures smooth transitions to Care Navigation with strong clinical assessment and communication skills.

Applicants must hold a Florida RN license, have at least 1 year of nursing experience (hospice/hospital preferred), and possess EMR/EHR experience.

Qualifications

• Current license as RN in the state where the employee will be working
• Minimum of one year nursing experience; hospice or hospital experience preferred
• Previous experience working with an EMR/EHR system
• Strong clinical assessment skills with preferred background in emergency department or ICU
• Ability to effectively communicate in English; bi-lingual in Spanish preferred
• Mobile Driver - Valid driver’s license and automobile insurance per Company policy

Responsibilities

• Completes clinical admissions for all Company services and informs patients/families about services
• Identifies admission objections and provides responses to concerns
• Educates patient/family on admission requirements and immediate needs
• Obtains physician orders and initiates Plan of Care
• Maintains professional relationships with facilities, physicians, and case managers
