Job summary

Chapters Health System is seeking an Admission RN to assess patients in residences or hospitals and initiate care transfers to Care Navigation. The role acts as an ambassador for the organization, coordinating services and ensuring smooth admissions.

The ideal candidate holds an RN license in the working state, with 1+ year nursing experience, hospice or hospital background preferred, and EMR/EHR familiarity. Strong assessment, communication, and teamwork are essential.

Qualifications

• Current RN license in the state of employment.
• Minimum 1 year nursing experience; hospice or hospital preferred.
• Experience with an EMR/EHR system.
• Strong clinical assessment skills; ED/ICU background preferred.
• Written and verbal English; bilingual in Spanish is a plus.
• Active BLS for healthcare professionals.

Responsibilities

• Completes clinical admissions for all Chapters Health System services.
• Explains services to patients and families, addressing fears and questions empathetically.
• Educates patients on admission requirements and immediate needs.
• Obtains physician orders and initiates Plan of Care.
• Initiates nursing interventions to prevent complications and maximize comfort.
• Maintains timely documentation and communicates with Care Navigation staff.
• Represents the company at assigned facilities and facilitates referrals.
• Ensures safe discharge planning and coordination with after-hours staff.
