Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to support patients in regaining strength and independence under the supervision of a licensed Occupational Therapist.

The role includes providing delegated therapy services, coordinating with the care team, driving to patient homes, and contributing to discharge planning. A valid driver's license and Home Health experience are preferred; pay is $41–$57 per visit.

Qualifications

• Adequate licensure as Occupational Therapy Assistant is assumed.
• Minimum six months occupational therapist assistant experience.
• Home Health experience preferred.

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist.
• Assist in the implementation of programs to restore, reinforce and enhance task performances.
• Design/adapt equipment to promote independence and participation in the program.
• Document observed information in patient records and coordinate care with the team.
• Participate in care coordination and discharge planning activities.
