CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to deliver therapy under the supervision of an Occupational Therapist, aiming to restore and improve patient independence in their homes.

The role involves implementing treatment plans, designing adaptive equipment, documenting progress, and coordinating care. Requirements include an OTA license, CPR, and a valid driver’s license; Home Health experience is preferred.

Qualificações

• OTA licensure current and unrestricted.
• CPR certification current.
• Valid driver’s license with auto insurance.

Responsabilidades

• Provide therapy services planned, delegated and supervised by the Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs established by the OT to restore function and independence.
• Design/adapt equipment and home environments to promote safety and independence.
• Fabricate devices to improve function and participation when possible.
• Provide therapeutic treatment and instruction to improve strength, coordination, ROM and functional tasks.
• Document observations in patient records and prepare clinical notes.
