CenterWell Home Health is seeking a Home Health Occupational Therapy Assistant to provide therapy services planned, delegated and supervised by an Occupational Therapist, delivering care in patients' homes to improve strength, mobility and independence.

The role requires OTA licensure, CPR, a valid driver's license and reliable transportation; prior home health experience is preferred. This full-time position includes benefits and 40 hours per week.

Aufgaben

• Provide therapy services planned, delegated and supervised by the Occupational Therapist.
• Assist in implementing programs to restore and enhance function and independence.
• Design/adapt equipment and environments to improve client participation and safety.
• Document observations in patient records and assist with progress reports.
• Instruct patients and family on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
