Job summary

CenterWell Home Health is seeking an Occupational Therapy Assistant to join our patient-centered home health team. You will deliver supervised therapy services, support in care coordination, and educate patients and families on home programs to promote independence.

Qualified candidates hold an OTA license, CPR certification, and a valid driver's license with reliable transportation. Prior home health experience is a plus; hours and pay vary by region.

Responsibilities

• Provide therapy services planned and supervised by the Occupational Therapist.
• Assist in implementing vocational/education programs and activities to restore, reinforce and enhance tasks and independence.
• Design/adapt equipment and living environments to improve function and independence.
• Provide therapeutic treatment and instruction per physician orders to improve strength, ROM and function.
• Document observations and progress in patient records and reports; maintain required documentation.
• Instruct patients and family on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Coordinate community reintegration activities with the team.
