Job summary

CenterWell Home Health is seeking a licensed Occupational Therapy Assistant to deliver therapy services in patients’ homes under the supervision of an Occupational Therapist. You will support rehab plans, help patients regain independence, and contribute to safe home environments. Qualifications include OTA licensure, CPR, and a valid driver’s license with reliable transportation.

Six months OTA experience and Home Health exposure are preferred. 40 hours/week.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver's license, auto insurance, and reliable transportation.
• Minimum six months occupational therapist assistant experience (preferred).
• Home Health experience (preferred).

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist.
• Assist in implementing vocational/education programs and activities.
• Design/adapt equipment and living environments to support function.
• Fabricate devices to aid independence and participation.
• Provide therapeutic treatment to improve strength, ROM and function.
• Record observations and contribute to patient records and progress notes.
• Assist with progress reports and required documentation.
