Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to join our caring team. You will work under the supervision of a licensed Occupational Therapist to deliver home-based rehabilitation, focusing on restoring strength, mobility and independence in patients in their own homes.

Key duties include implementing therapy plans, designing adaptive environments, documenting progress, and teaching patients and families.

Qualifications

Current OTA licensure (unrestricted).
Current CPR certification required.
Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

Provide therapy services planned, delegated and supervised by the Occupational Therapist.
Assist in implementing vocational/education programs and activities.
Design/adapt equipment and working/living environments.
Fabricate devices to assist and improve function and independence.
Provide therapeutic treatment and instruction per physician orders to improve strength and ROM.
Document observations in patient records and prepare clinical notes.
Assist with progress reports and maintain required documentation.
Instruct patients and families on home programs and use of adaptive equipment.
Participate in care coordination and discharge planning with the interdisciplinary team.
