Job summary

CenterWell Home Health is seeking a Home Health Occupational Therapist Assistant to deliver personalized therapy at patients' homes. You will work under the supervision of a licensed Occupational Therapist, helping patients regain strength and independence through targeted rehabilitation activities and home programs.

The role requires OTA licensure, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

Current and unrestricted OTA licensure.
Current CPR certification.
A valid driver's license, auto insurance, and reliable transportation.
Preferred: minimum six months occupational therapist assistant experience.
Preferred: Home Health experience.

Responsibilities

Provide therapy services planned, delegated and supervised by the OT in accordance with the patient's Plan of Treatment.
Assist in implementing vocational/education programs designed to restore and enhance function.
Design/adapt equipment and environments to improve independence.
Provide therapeutic treatment and instruction to improve/restore strength, coordination and ROM.
Document patient records and assist with progress reports.
Instruct patients and families on home programs and use of adaptive equipment.
Coordinate care and discharge planning with the interdisciplinary team.
