Job summary

CenterWell Home Health is seeking a compassionate Occupational Therapy Assistant to provide home-based therapy under the supervision of an Occupational Therapist. You will support patients in regaining independence, executing home programs and adapting environments to improve daily living activities.

The role requires OTA licensure, CPR certification, and a valid driver's license with insurance. Prior home health experience preferred; 40 hours weekly with competitive pay and benefits.

Qualifications

• Current OTA licensure in good standing.
• CPR certification active and current.
• Valid driver's license and auto insurance required.

Responsibilities

• Provide therapy services planned, delegated and supervised by the Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs and activities to restore and enhance function.
• Design/adapt equipment and living environments to improve independence and participation.
• Deliver therapeutic treatment to improve strength, ROM, coordination and function per physician orders.
• Document observations in patient records and assist with progress reports and required documentation.
• Instruct patients and families on home programs and use of adaptive equipment.
• Participate in care coordination, discharge planning and interdisciplinary teamwork.
• Plan community reintegration activities to support physical reconditioning and psychological adjustment.
