CenterWell Home Health is seeking a licensed Occupational Therapy Assistant to provide patient-focused therapy in home settings. Services are planned and supervised by an Occupational Therapist, aimed at restoring independence and functional ability.

The position requires OTA licensure, CPR certification and a valid driver’s license with auto insurance. Prior home health experience is preferred, with a typical full-time schedule of 40 hours per week.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• Valid driver's license and auto insurance.

Responsabilités

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
• Assist in implementing vocational/education programs and activities established by registered Occupational Therapist to restore and enhance task performances.
• Design/adapt equipment and living environments to improve function and independence.
• Provide therapeutic treatment and instruction per physician orders to improve/restore strength, coordination, ROM and function.
• Document patient records, progress notes and assist with progress reports as required.
• Instruct patients and family members on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Coordinate community reintegration activities to support physical reconditioning and psychological adjustment.
