Job summary

CenterWell Home Health seeks a Home Health Occupational Therapist Assistant to support patients at home under supervision. You will deliver therapy, modify equipment, and educate patients and families to improve independence and function.

The role requires OTA licensure, CPR, and a valid driver’s license; you will coordinate with the care team, document progress, and participate in discharge planning. 40-hour weeks with pay between $64,000 and $87,500 annually.

Qualifications

• Current and unrestricted OTA licensure.
• Current CPR certification.
• A valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist.
• Assist in implementing vocational/education programs established by the OT.
• Design/adapt equipment and the home environment to improve function.
• Provide therapeutic treatment and instruction to improve strength, ROM and function.
• Document observations in patient records and assist with progress reports.
• Instruct patients and families on home programs and use of adaptive equipment.
• Participate in care coordination and discharge planning with the interdisciplinary team.
• Design community reintegration activities to support physical reconditioning and psychological adjustment.
