Job summary

ADAPT Community Network in New York City seeks a Residence Program Specialist to support residents in ADAPT’s residential facilities. You will promote independence, provide direct care, and advocate for individuals to achieve personal goals while maintaining a safe, home-like environment.

The role requires AMAP certification or eligibility, NY driver’s license, and strong communication with residents, families, and staff to ensure high-quality, person-centered care.

Qualifications

• High School Diploma or GED required.
• AMAP at time of hire or within six months.
• Valid New York State Driver's License.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills.

Responsibilities

• Provides direct support and helps residents develop skills for greater independence.
• Ensures safety, health, and well-being of residents at all times.
• Teaches activities of daily living and community living skills.
• Maintains a homelike, respectful atmosphere in the residence.
• Participates in interdisciplinary treatment plans and implements goals.
• Documents daily activities, progress notes, and required records accurately.
• Responds to crises and medical emergencies, including first aid.
• Administers medications as AMAP under nursing supervision.
• Provides transportation for residents to programs and appointments.
• Attends staff meetings and trainings to support quality care.
