Job summary

ADAPT Community Network is seeking a Residence Program Specialist to support individuals in a residential facility in New York City. You will assist with daily living skills, safety, and personalized program goals while maintaining a dignified, homelike atmosphere.

Responsibilities include medication administration under supervision, crisis response, documentation, and transportation support.

Qualifications

• High School Diploma or GED required or preferred.
• AMAP authorization at hire or within six months.
• Valid New York State Driver's License.
• Ability to read, write, speak and understand English.
• Strong interpersonal and communication skills for working with people we support, families, coworkers, and community partners.
• Ability to respond to crisis situations and make sound decisions under pressure.
• Commitment to confidentiality and safeguarding rights of people supported.
• Ability to work independently and as part of a team.

Responsibilities

• Provides care and supports people we support in achieving greater independence and personal development.
• Ensures safety, health, and well-being of the people we support at all times.
• Teaches daily living activities and life skills.
• Maintains a homelike, respectful atmosphere within the residence.
• Documents daily activities, progress notes, and records accurately.
• Responds to crisis situations and medical emergencies with first aid and coordination with emergency personnel.
• Administers medications as AMAP under nursing supervision.
• Provides transportation to program, work, and appointments.
• Assists with meal planning, shopping, and meal preparation.
• Attends staff meetings and trainings to support quality care.
