Job summary

Evergreen Healthcare Group is seeking a Resident Care Manager Nurse (RCM) to oversee total care for residents at Granite Rehabilitation and Wellness in Cheyenne, WY. The role includes supervising licensed nurses and CNAs and ensuring adherence to nursing processes for geriatric residents in long-term care and rehabilitation settings.

The candidate will manage medications, monitor resident conditions, document changes, and coordinate with families and physicians.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN or LPN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.

Responsibilities

• Obtain and administer prescribed medications and treatments per nursing techniques.
• Observe residents, document changes, and notify supervisor, family, and physician of health changes.
• Take vital signs to detect deviations and assess condition.
• Provide appropriate disciplinary action for nursing staff non-compliance with regulations and policies.
• Update care plans and ADL sheets as residents’ conditions change.

Job Summary:

Granite Rehabilitation and Wellnessis currently looking for aResident Care Manager Nurse (RCM) to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Graduate of an accredited school of nursing.
• Current RN or LPN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
