Job summary

Evergreen Healthcare Group is seeking a Resident Care Manager (RCM) for the Shepherd of the Valley Rehabilitation and Wellness facility in Casper, WY. This on-site leadership role oversees nursing staff and ensures delivery of total resident care, coordinating with RNs and CNAs to maintain high standards of geriatric care.

Required: graduation from an accredited nursing program and current RN or LPN licensure in Wyoming.

Qualifications

• Graduate of an accredited school of nursing.
• Current LPN or RN licensure in the state of employment.
• Knowledge of nursing process in geriatric, long-term care, and rehabilitation.

Responsibilities

• Administer medications and treatments following approved nursing techniques.
• Observe residents, record significant conditions, and notify supervisor, family, and physician of changes.
• Take vital signs to detect deviations and assess condition.
• Administer disciplinary action for nursing staff non-compliance with regulations and policies.
• Modify resident care plans and ADL/flow sheets with changes in condition.

Job Summary:

Shepherd of the Valley Rehabilitation and Wellnessis currently looking for aResident Care Manager (RCM)to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required:Graduate of an accredited school of nursing.
• Required:Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
