Job summary

Granite Rehabilitation and Wellness is currently seeking a Resident Care Manager Nurse (RCM) to oversee the delivery of comprehensive nursing care for residents. The RCM will supervise Licensed Nurses and CNAs while ensuring adherence to care plans and regulatory requirements.

The role emphasizes medication administration, observation of resident conditions, vital signs monitoring, and coordination with families and physicians.

Qualifications

• Graduate of an accredited school of nursing.
• Current RN or LPN licensure in the state of employment.
• Knowledge of nursing process in geriatric residents, long-term care, and rehabilitation.

Responsibilities

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, responsible party and physician of changes.
• Takes vital signs to detect deviations from normal and assess condition.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, ADL and Aide flow sheets with resident changes in condition.

Job Summary:

Granite Rehabilitation and Wellnessis currently looking for aResident Care Manager Nurse (RCM)to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required:Graduate of an accredited school of nursing.
• Required:Current RN or LPN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
