Job summary

Evergreen Health­care Group is seeking a Resident Care Manager Nurse (RN or LPN) at Shelton Health and Rehabilitation Center in Shelton, WA. Wage depends on experience and licensure. The role oversees nursing staff and ensures proper care to residents.

Responsibilities include administering medications, monitoring residents, updating care plans, and ensuring regulatory compliance with state and federal guidelines. A nursing degree and WA license are required.

Qualifications

• Must be a graduate of an accredited nursing program.
• Current LPN or RN licensure in WA.
• Knowledge of nursing process in geriatric, long-term care, and rehab settings.

Responsibilities

• Observe residents, document changes, and inform supervisor, family, and physician.
• Monitor vital signs and respond to deviations.
• Administer disciplinary action for staff non-compliance with regulations and policies.
• Update care plans and ADL sheets as residents’ conditions change.

Job Summary:

Shelton Health and Rehabilitation Center is currently looking for aResident Care Manager Nurseto oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.

• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.

• Takes vital signs to detect deviations from normal and assess condition of resident.

• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.

• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required:Graduate of an accredited school of nursing.

• Required:Current LPN or RN licensure in the state of employment.

• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
