Job summary

Evergreen Healthcare Group seeks a Resident Care Manager (RCM) in Rawlins, WY to oversee total resident care and supervise Licensed Nurses and CNAs. You will provide nursing care, monitor treatment plans, and ensure compliance with state and federal regulations.

The role requires nursing licensure (RN or LPN) in Wyoming and graduation from an accredited nursing program. Strong leadership, clinical judgment, and communication with families are essential.

Qualifications

• Knowledge of nursing process, geriatric care, long-term care and rehabilitation.
• Ability to oversee Licensed Nurses and CNAs.
• Strong record-keeping and communication with families and physicians.

Responsibilities

• Administer medications and treatments per nursing standards.
• Observe residents, document changes, and notify supervisors and families.
• Measure vital signs and respond to deviations in condition.
• Enforce nursing staff compliance with regulations and policies.
• Update care plans, ADLs, and care directives as conditions change.

Job Summary:

Rawlins Rehabilitation and Wellnessis currently looking for a Resident Care Manager (RCM)to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required:Graduate of an accredited school of nursing.
• Required:Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
