Job summary

Worland Health and Rehabilitation in Worland, WY is seeking a Resident Care Manager (RCM) to oversee the administration of total care to residents and supervise Licensed Nurses and CNAs. The role includes managing medications, monitoring conditions, and coordinating care plans in a long-term care setting.

The RCM will ensure nursing processes align with geriatric and rehabilitation needs, perform vital signs, and address changes with supervision and physician communication.

Qualifications

• Graduate of an accredited school of nursing.
• Current LPN or RN licensure in the state of employment.
• Knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.

Responsibilities

• Obtains and administers prescribed medications and treatments.
• Observes resident and records conditions; notifies supervisor, family, and physician of changes.
• Takes vital signs to detect deviations and assess condition.
• Administers disciplinary action for staff non-compliance with regulations and policies.
• Modifies care plans and ADL/flow sheets in response to resident changes.

Job Summary:

Worland Health and Rehabilitation is currently looking for a Resident Care Manager (RCM) to oversee the proper administration of total care to our residents. Provides general nursing care to residents, and oversee the work of the Licensed Nurses and CNAs in our facility.

Essential Functions:

• Obtains and administers prescribed medications and treatments in accordance with approved nursing techniques.
• Observes resident, records significant conditions and reactions, and notifies supervisor, family, (responsible party) and physician of resident's change in condition and reaction to drugs, treatments, and significant incidents.
• Takes vital signs to detect deviations from normal and assess condition of resident.
• Administers appropriate disciplinary action for nursing staff non-compliance with state and federal regulations and Center policies.
• Modifies resident care plan, care directives, Activities of Daily Living (ADL) and Aide flow sheets in conjunction with resident changes in condition

Education, Licensure, and Experience:

• Required: Graduate of an accredited school of nursing.
• Required: Current LPN or RN licensure in the state of employment.
• Demonstrated knowledge of nursing process, particularly as it relates to geriatric residents, long-term care, and rehabilitation.
