Job summary

PACS is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care to residents in our post-acute care setting. You will follow care plans and work under supervision to support residents' daily activities.

Responsibilities include identifying residents, recording observations, maintaining hygiene, assisting with meals and mobility, and reporting changes to the Nurse Supervisor. CPR preferred; 10th-grade education minimum.

Qualifications

• Must possess a valid Certified Nursing Assistant license as required by state law.
• CPR certification preferred.
• Minimum education includes a 10th-grade education.

Responsibilities

• Identify residents before treatments using wristbands or photo IDs.
• Record entries accurately on flow sheets and charts.
• Maintain residents' rooms and comfort; assist with transfers.
• Report changes in residents' condition to the Nurse Supervisor/Charge Nurse.
• Protect resident confidentiality and report breaches immediately.
