Job summary

PACS is seeking a licensed Certified Nursing Assistant to provide routine daily nursing care to residents according to care plans. You will identify residents before treatments or meals, document entries accurately, and report changes in condition to the Nurse Supervisor.

Qualifications

• Must be a licensed Certified Nursing Assistant (CNA) per state law.
• CPR certification preferred but not required at hire.
• Ability to read, write, and follow care plans and documentation guidelines.

Responsibilities

• Provide routine daily nursing care per resident assessments and care plans.
• Identify residents before delivering treatments or meals.
• Record entries accurately on flow sheets, notes, and charts.
• Report changes in resident condition to supervisor promptly.
• Maintain confidentiality of resident information and report any disclosure violations.
• Assist with daily living activities and mobility as trained.
• Ensure rooms are prepared for residents and assist with transfers or admissions.
• Support a calm, warm environment and participate in shift nursing reports.
