Job summary

Sweetwater Care in Ohio is seeking an Activities Assistant to support the Activities Director by creating a monthly calendar, coordinating resident activities, and helping with attendance and documentation.

You will assist with getting residents to activities, support daily living tasks as outlined in care plans, and collaborate with other staff to maintain a safe, engaging program for residents.

Qualifications

• High school diploma or equivalent.

Responsibilities

• Assist in creating a monthly calendar of activities written in large print and posted for residents/visitors.
• Assist in placing a copy of the calendar in each resident’s room.
• May interview residents or families to obtain activity preferences.
• Assist residents in getting to activity locations within the facility.
• Conduct activities to promote physical, social, and mental well-being of residents.
• Assist with activities of daily living for residents per care plans and procedures.
• Assist with dining room setup, delivery, feeding, and documenting meal intake.
• Document activity attendance and resident responses in progress notes.
• Observe resident mood and involvement; report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Assist in in-room activities for one-on-one visits when needed.
• Coordinate offsite field trips and transportation as required.
• Coordinate activity needs with other departments to prevent interruptions.
• Support Resident/Family Council meetings as directed.
• Report changes in resident condition to nursing staff.
