Job summary

Sweetwater Care in Fowler, CA is seeking an Activities Assistant to support the Activities Director in planning a monthly calendar of activities and posting it in large print in a prominent location.

Responsibilities

• Assist Activities Director in creating a monthly calendar of activities.
• Post the calendar in large print in a prominent location visible to residents and visitors.
• Ensure a copy of the calendar is placed in each resident's room.
• Interview residents or family members to obtain activity preferences.
• Coordinate transportation and assist residents in getting to activity locations.
• Assist in conducting scheduled activities to promote physical, social, and mental well-being.
• Document attendance and ensure charted progress notes describe services and resident responses.
• Observe mood and participation and report changes to the Activities Director.
• Involve residents and families in planning activities when possible.
• Assist with in-room activities for one-on-one visits.
• Coordinate and arrange offsite field trips and transportation as needed.
• Coordinate activities with other departments to prevent interruptions.
• Assist with Resident/Family Council meetings as indicated.

Education
