Qualifications

• High school diploma or equivalent required or preferred.

Responsibilities

• Assist Memory Care Director in creating a monthly activities calendar written in large print and posted visibly.
• Assist in posting monthly activity calendars in residents' rooms.
• Interview residents or families to obtain activity preferences.
• Assist residents in getting to activity locations within the facility.
• Conduct activities to promote physical, social, and mental well‑being.
• Document attendance and chart progress notes with resident response.
• Observe mood and engagement; report changes to Activities Director.
• Involve residents and families in planning activities when possible.
• Support in-room activities for one-on-one residents.
• Coordinate offsite field trips and transportation as needed.
• Coordinate needs with other departments to avoid interruptions in activities.
• Assist with Resident/Family Council meetings as directed.
• Maintain dignity and residents’ rights; follow facility policies and procedures.
• Follow safety/hygiene measures and protect health information.
