Job summary

Ohio Living Swan Creek is seeking an Activities Coordinator to plan and direct the activity department, ensuring programs support residents’ well-being and comply with standards. The role requires coordinating with the care team, maintaining documentation, and staying informed on licensure and reimbursement issues.

Qualifications

• High school diploma or equivalent required.
• Bachelor’s degree in recreation, art therapy, or related field preferred.
• Certified Therapeutic Recreational Specialist preferred.
• Two years experience in a long-term care, hospital, or related healthcare setting required.
• Must read/write/speak English.

Responsibilities

• Plan, organize, and direct activity programs for residents to support physical, mental, and psychosocial well-being.
• Maintain charts, notes, and documentation for compliance with regulations.
• Interact with residents and families, addressing requests and concerns.
• Attend care conferences and coordinate with other departments as needed.
