Job summary

Sweetwater Care is seeking an Activity Assistant to support the planning, implementation, and evaluation of recreational, social, and spiritual programs for residents. The role requires a positive attitude, reliability, and the ability to work with a care team to enhance daily life for residents.

You will assist in coordinating activities, observing resident engagement, and helping families participate in planning goals, with opportunities to contribute to quality assurance and care planning.

Qualifications

• Must have a positive attitude and patience with residents.
• Experience preferred but not necessary.
• Dependable, energetic, self-starter with strong organization skills.

Responsibilities

• Assist in planning, implementing, and evaluating activity programs for residents.
• Observe resident attendance, mood, behavior, and involvement for progress.
• Coordinate activities with other departments and arrange transportation when needed.
• Interview residents or family members to obtain activity information.
• Involve residents and families in planning objectives and goals.
• Complete necessary forms and reports for the Activity Director.
• Refer residents to appropriate social services when needed.
• Maintain charted activity notes describing resident responses.
