Job summary

Highmark Health in Pennsylvania seeks a wellness/lifestyle coach to own a panel of members, assess health needs, and tailor interventions. You will work with a multidisciplinary team to create personalized care plans and guide members toward improved health outcomes across telephonic, digital, or virtual channels.

You will manage care coordination, track progress, and ensure documentation meets regulatory and quality standards while supporting program pilots and enhancements.

Qualifications

• Master's degree in nutrition or related field or equivalent experience.
• Minimum 3 years as RD/Exercise Physiologist or Wellness/Lifestyle Coach.
• Knowledge of HIPAA and compliance in healthcare settings.

Responsibilities

• Provide lifestyle coaching across tobacco cessation, weight, nutrition, sleep, stress, and activity.
• Develop care plans and remove barriers; coordinate internal/external resources.
• Conduct outreach to members via phone, digital, or virtual platforms.
• Document activities in compliance with business, regulatory and accreditation standards.
• Collaborate in workgroups to improve program efficiency and member focus.
• Other duties as assigned.
