Mindoula Health is seeking a Washington Certified Peer Support Specialist to support members across Washington State. This role is primarily remote with occasional community-based work about 20% of the time.

You must be fully certified as a WA Peer Support Specialist to be considered for this opening. The role involves following Substance Use Disorder Program guidelines to plan care, assess client needs, and complete screenings.

• Must be Washington-certified Peer Support Specialist.

Responsabilidades

• Identifying member needs and documenting interventions based on relationships and assessments.
• Providing support to members through a variety of settings and assisting with support groups as needed.
• Contributing to case management meetings to ensure a team approach for each member.
• Completing documentation for member contacts, touches, cancellations and care plans per policy.
• Communicating professionally with members, families, physicians, community agencies, and staff.
• Following chain of command in communicating concerns or recommendations.
• Achieving set goals/KPIs and promoting Mindoula and SUD program through outreach.
• Participating in performance improvement processes and promoting responsible use of Mindoula resources.
