Job summary

CVS Health Minute Clinic is seeking a remote Care Nurse Practitioner to deliver holistic, evidence-based care via a telehealth platform. You will collaborate with a dedicated team and independently manage assessments, diagnoses, treatments, and disposition planning for patients aged 18 months and older.

The role requires an active NP license, a Master's in Family NP, and at least two years of related experience.

Qualifications

Active, unrestricted licensure in any US state/district.
Minimum of two years of medically-relevant experience or equivalent.
Effective verbal, written, and electronic communication skills.
Ability to work independently and practice autonomously.

Responsibilities

Provide holistic, evidence-based care including assessment, diagnosis, treatment, management of health problems, counseling, and disposition planning for patients aged 18 months and up.
Evaluate primary, acute, chronic, and transitional care patients and educate patients accordingly.
Counsel patients on pregnancy prevention, STI prevention, safer sex practices, and contraception.
Educate patients on health maintenance and respond to inquiries.
Document all patient care within an EHR per policy.
Coordinate care with internal and external colleagues in a patient-centered medical home.
Collaborate with Providers, physicians, paraprofessionals, pharmacists and others to deliver high-quality care.
Work independently, prioritize and advocate for patients.
