Job summary

CVS Health Minute Clinic seeks a qualified Family Nurse Practitioner to deliver holistic, evidence-based care via remote technology. The role serves patients 18 months and older, with responsibilities including assessment, diagnosis, treatment, counseling, and care coordination.

Ideal candidates hold an active FNP license, a Master’s degree, and national board certification, with 2+ years of experience. The position offers a full-time schedule (12:00 p.m.

Qualifications

• Candidates must be currently licensed in one of the states listed (CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, WV).
• Minimum of two years of medically-relevant experience or equivalent.
• Effective verbal, written, and electronic communication skills.
• Outstanding organizational skills and ability to multi-task.
• Initiative, problem solving ability, adaptability and flexibility.
• Ability to work without direct supervision and practice autonomously.
• Is proficient with information management and technology.
• Capacity to collaborate with professional colleagues as necessary to provide quality care.
• Depending on the market, the ability to be proficient in additional languages (e.g., Spanish) may be required.

Responsibilities

• Provide holistic, evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients aged 18 months and above.
• Evaluate primary care, acute, chronically ill, and transitional care patients, with healthcare education and counseling.
• Provide patient counseling; contraception and STI prevention/safer sex practices.
• Educate patients on health maintenance and respond to patient care inquiries.
• Document all patient care within an EHR according to MinuteClinic policies and procedures.
• Coordinate care with internal and external colleagues to provide high-quality care.
• Effectively work within a patient care team with Providers, Collaborative Physicians, paraprofessionals, Pharmacists and others.
• Work independently, prioritize and take initiative, and advocate for patients and their practice.
