Qualifications

• Active, unrestricted licensure in any US state or district.
• Minimum of two years of medically-relevant experience.
• Strong verbal, written, and electronic communication skills.
• Excellent organizational and multitasking abilities.
• Ability to work autonomously with limited supervision.
• Proficient with information management and EHR/tech systems.
• May require bilingual abilities (e.g., Spanish) depending on market.

Responsibilities

• Deliver holistic, evidence-based care to patients aged 18 months and older via remote technology.
• Assess, diagnose, treat, and manage health problems; provide counseling and disposition planning.
• Educate patients on health maintenance and preventive care, including pregnancy prevention and STI guidance.
• Document care accurately in the EHR per policies.
• Collaborate with clinicians and care teams to coordinate patient care.
