Job summary

United States Digital Space LLC in the United States is seeking a Care Navigation and Benefits Specialist to help Veterans navigate healthcare, benefits, behavioral health, and community resources in a fully remote role. The role includes guiding complex Veteran cases, improving workflows, training care teams, and shaping how care coordination is delivered as the program grows.

5+ years of VA or veteran services experience is preferred, with remote work experience and LMSW-like credentials

Qualifications

• 5+ years of experience as a clinical social worker, care coordinator, care navigator, or in a similar field, with demonstrated progressive program management experience.
• Direct experience working with Veterans and/or within the VA system.
• 1+ years of experience working successfully in a remote environment.
• Knowledge of VA healthcare, Veteran benefits and resources, behavioral health, and social determinants of health.
• Experience documenting cases within a care management or electronic health record system.

Responsibilities

• Help Veterans navigate healthcare, benefits, behavioral health services, community resources, and other support.
• Serve as a resource to other care coordinators and help improve Veteran-specific workflows.
• Identify gaps in the program and contribute to training.
• Help shape how care coordination is delivered as the program develops.
