Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist (Remote) to support Veterans by guiding them through VA healthcare, benefits, and community resources. The role emphasizes collaboration with care teams, program development, and escalation when needed.

Ideal candidates bring extensive Veteran/VA experience, strong care coordination skills, and at least five years in related roles, with remote-work experience. This full-time position offers a remote schedule within Central Time zone.

Qualifications

• 5+ years in clinical social work, care coordination, or related field with progressive program management.
• Direct experience with Veterans or VA system.
• Remote work experience (1+ year).
• Knowledge of VA healthcare, benefits and resources.
• Experience with complex medically/behaviorally diverse populations.
• Experience documenting cases in an electronic health record system.

Responsibilities

• Help Veterans understand and navigate VA healthcare, benefits, community care, and related services.
• Guide complex Veteran cases, determine referrals and escalations as needed.
• Help develop Veteran-specific care coordination workflows and identify improvement opportunities.
• Collaborate with training leadership to strengthen the team's understanding of VA resources and barriers to care.
• Serve as a resource to other care coordinators and support program development.
