Job summary

Volunteers of America Chesapeake & Carolinas seeks a Health Navigator to manage crises, support veterans with complex behavioral health needs, and connect clients to quality care and VA services.

The role focuses on intake-to-housing pathways, providing day-to-day support for veteran households in homelessness prevention and rapid re-housing programs, with telework capacity and regional travel as needed.

Qualifications

• Master's level social work or equivalent education with experience in counseling psychology, nursing, rehabilitative counseling, or related human service field.
• Experience working with homeless individuals and families including veterans is a plus.
• Comfort in environments with constantly evolving responsibilities.
• Passion for connecting individuals with excellent care and able to provide short-term support in person, over the phone and via email.

Responsibilities

• Coordinate efforts for the SSVF Program; visit VA, shelters, and other places where veterans congregate; maintain updates with VA and housing providers.
• Manage client crises with brief risk assessment, safety planning, and de-escalation; connect clients to therapeutic services.
• Independently manage a caseload of complex, multi-need clients; conduct housing assessments and develop Family Services Plans.
• Provide service coordination between local VAMC and community partners; revise service plans and develop placement, stabilization and follow-up.
• Travel to housing sites to support clients, monitor progress and address health, behavioral or life skills issues.
• Document client interactions and ensure timely file documentation; participate in supervision and program meetings.
