Job summary

Senior Services of Southeastern Virginia seeks a Veterans Directed Care (VDC) Options Counselor for a fully remote, home-office-based role serving veterans and caregivers. You will assess needs, develop individualized care and spending plans, and provide ongoing support while traveling up to 50% within the service area and making monthly visits to the Norfolk headquarters.

Qualifications

• Bachelor's degree in social work or related field, or two years of relevant community-based experience.
• Knowledge of aging, disability, veteran benefits, Medicaid/Medicare preferred.
• Strong ability to research and connect individuals with community resources and services.
• Excellent listening, interviewing, assessment, communication, and relationship-building skills.
• Ability to empower veterans and caregivers with a person-centered approach.
• Strong organizational and time-management skills.
• Ability to work independently and manage changing priorities.
• Proficiency with Microsoft Office, especially Excel, and web-based systems.
• Maintain confidentiality and comply with HIPAA.

Responsibilities

• Complete initial assessments and required documentation.
• Identify needs and suitable services with veterans and caregivers.
• Develop and monitor individualized care and spending plans.
• Help participants understand responsibilities and available services.
• Provide guidance, training, and ongoing support.
• Conduct monthly, quarterly, semi-annual, and annual contacts.
• Follow up on referrals and ensure services are implemented.
• Maintain confidential participant records in electronic databases.
• Monitor participant spending and complete required reports.
• Coordinate with healthcare providers and community partners.
• Build and maintain relationships with veterans, caregivers, professionals, and partners.
• Provide education and presentations to groups.
• Use Cumulus and other web-based systems to maintain records.
