Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist for a remote, full-time role focused on Veterans’ healthcare navigation, benefits, and community resources. You will support care coordinators, enhance workflows, and help shape how care coordination is delivered as the program grows.

Your background in Veteran care, care coordination, and increasing program responsibility will be highly valued, with remote work experience and knowledge of VA resources.

Qualifications

• 5+ years as a clinical social worker or care navigator with progressive program responsibilities.
• Direct experience with Veterans/VA system.
• 1+ year remote work experience.
• Knowledge of VA healthcare, benefits, behavioral health, and social determinants of health.
• Experience documenting cases in a care management/EHR system.

Responsibilities

• Help Veterans navigate VA healthcare, benefits, community resources, and social needs.
• Guide complex Veteran cases, determine referrals and escalations, provide direct support as needed.
• Develop Veteran-specific care coordination workflows and improve program operations.
• Collaborate with care coordination and training leadership to strengthen VA knowledge and resources.
