Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist (Remote) to help Veterans navigate VA healthcare, benefits, behavioral health services, and community resources. You will support the care team, manage complex cases, and shape workflows as the program grows.

The role emphasizes remote collaboration, training, and Veteran-focused process improvement. Candidates with VA familiarity and care coordination experience are encouraged; Spanish helpful but not required.

Qualifications

5+ years as clinical social worker, care coordinator, or similar with progressive program management.
Direct experience with Veterans or within the VA system.
Remote work experience in the past year.
Knowledge of VA healthcare, benefits, behavioral health and social determinants of health.
Experience documenting cases in a care management or EHR system.

Responsibilities

Guide complex Veteran cases and determine referrals and escalations.
Develop Veteran-specific care coordination workflows and training materials.
Collaborate with care coordination and training leadership to strengthen VA knowledge.
Be a resource to other care coordinators and improve program workflows.
Support the broader team in handling complex situations and care delivery.
