Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist to support Veterans remotely. You will leverage VA knowledge to help navigate healthcare, benefits, behavioral health, and community resources while guiding care coordinators and shaping workflows.

Required are 5+ years in clinical social work or care coordination, direct VA experience, and strong remote-work capabilities. Salary ranges from $80,000 to $100,000 with Central Time-based hours.

Qualifications

• 5+ years in clinical social work or care coordination with progressive program management
• Direct experience with Veterans and/or VA system
• 1+ year remote work experience
• Knowledge of VA healthcare, benefits, behavioral health, and social determinants of health
• Experience documenting cases in a care management or EHR system
• Strong communication, collaboration, and problem-solving skills
• Experience with motivational interviewing is a plus
• Spanish language helpful but not required

Responsibilities

• Help Veterans navigate VA healthcare, benefits, community resources, and other services
• Guide complex Veteran cases, referrals and escalations
• Develop veteran-specific care coordination workflows and train leadership
• Identify opportunities to improve the program and strengthen VA knowledge among the team
• Support care coordination staff and provide remote expertise to colleagues
