Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist for a remote role focused on veteran-focused care coordination. You will help Veterans navigate VA healthcare, benefits, and community resources while supporting the care team in complex cases.

The role emphasizes developing veteran-specific workflows, identifying program gaps, and contributing to training. Experience with VA systems and remote work is highly valued.

Qualifications

• 5+ years of experience in clinical social work, care coordination, or care navigator roles.
• Direct experience with Veterans and/or VA system.
• 1+ year of remote work experience.
• Knowledge of VA healthcare, Veteran benefits, behavioral health, and social determinants of health.

Responsibilities

• Help Veterans navigate healthcare, benefits, behavioral health services, community resources, and support.
• Serve as a resource to other care coordinators and help shape workflows.
• Develop Veteran-specific care coordination workflows and identify program gaps.
• Train and support care coordination leadership to strengthen VA healthcare knowledge.
• Support complex Veteran cases and determine referrals and escalations.
