Job summary

AlediumHR is seeking a Care Navigation and Benefits Specialist (Remote) to assist Veterans in navigating healthcare, benefits, and community resources. You’ll support complex cases, developVeteran-specific workflows, and train teams to improve care coordination.

Qualified candidates bring Veteran or VA experience, care coordination background, and remote-work capability, with strong communication and problem-solving skills.

Qualifications

• 5+ years in clinical social work, care coordination, or similar with progressive program management.
• Direct experience working with Veterans and/or within the VA system.
• 1+ year remote work experience.
• Knowledge of VA healthcare, Veteran benefits, behavioral health, and social determinants of health.
• Experience supporting medically and/or behaviorally complex populations.
• Experience documenting cases in a care management or EHR system.
• Strong communication, collaboration, and problem-solving skills.
• Motivational interviewing experience is important.
• LMSW or similar credential encouraged; license not required.
• Fluent Spanish helpful but not required.

Responsibilities

• Help Veterans navigate VA healthcare, benefits, and community resources.
• Guide complex Veteran cases, determine referrals and escalations.
• Develop Veteran-specific care coordination workflows and improve program.
• Collaborate with care coordination and training leadership to strengthen VA knowledge.
