Job summary

86Borders, LLC. is seeking a remote Care Coordinator specializing in veteran benefits, resources, and protocols. The role focuses on navigating VA care, community resources, and social determinants of health to support Veterans, with supervision by the Director of Care Coordination.

The position requires extensive experience with VA systems, benefits education, and the ability to work effectively in a multi-cultural setting, primarily via home-office remote work.

Qualifications

• 5+ years as a clinical social worker, care coordinator, care navigator or similar field with progressive program management experience.
• 1+ years of experience working remotely.
• Experience with Veterans and/or within the VA system.
• Knowledge of VA healthcare, veteran benefits/resources, behavioral health, and SDOH.
• Experience with medically/behaviorally complex populations.

Responsibilities

• Program and Workflow Development: develop veteran-specific care coordination workflows and processes; establish guidelines for complex cases and escalations; identify scopes for non-clinical staff; identify gaps and opportunities.
• VA Resource Subject Matter Expert: serve as resource regarding VA healthcare, benefits, and veteran-specific services; navigate VA, community, and veteran resources; provide benefits education.
• Care Coordination Support: guidance on complex veteran cases; determine referrals/resources/escalation; provide direct veteran support.
• Training and Team Support: partner to develop veteran-specific training; educate CCs on VA healthcare and benefits navigation; act as SW/VA SME for CC team.
