Job summary

i4 Search Group Healthcare Recruiting is seeking an experienced Veteran Care Coordination Specialist (Remote). You will navigate VA care, Community Care, veteran benefits, behavioral health resources, and social determinants to support veterans across Alabama, Mississippi, Louisiana, or Tennessee.

Key duties include developing workflows, guiding care coordinators, educating veterans on benefits, and maintaining up-to-date knowledge of veteran programs.

Qualifications

• 5+ years in clinical social work, care coordination, or related roles.
• Experience with veterans, VA healthcare, or veteran benefits.
• Remote work readiness and ability to work independently.
• Strong communication and collaboration with multidisciplinary teams.
• Experience documenting care in electronic health records.

Responsibilities

• Develop and improve veteran-specific care coordination workflows and processes.
• Guide non-clinical care coordinators, nurses, and social workers on scopes of practice.
• Navigate VA systems, community resources, and veteran services for members.
• Educate veterans and team members about available benefits and resources.
• Document interactions and care activity in care management/EHR platforms.
• Support training materials and ongoing education for the care team.
• Identify gaps and opportunities to improve access to care and services.
