• Financial Management: Monitor financial performance related to value-based care contracts and identify opportunities for cost savings and revenue enhancement.
• Patient Engagement: Develop strategies to involve patients in their care and treatment plans; implement education programs to promote health and reduce avoidable utilization.
• Technology Utilization: Evaluate and recommend tech solutions, including EHR optimization, to support value-based care initiatives.
