Oak Street Health seeks a Clinical Call Center Triage Nurse to extend care beyond regular clinic hours by addressing patients\' medical, social, and psychological needs over the phone. You will deliver confident patient service, document meticulously, and coordinate appointments and transportation to support continuity of care.

Qualifications

• Active RN license in OSH practice states.
• Proven ability to manage time and work autonomously.
• Experience with telehealth or call-center nursing is a plus.

Responsibilities

• Provide confident and professional service to OSH patients.
• Follow current Prescription Refill policy.
• Document clearly during calls and ensure complete records before ending a call.
• Schedule appointments for patients and arrange transportation as needed.
• Monitor CareReq queue and ensure tasks are completed within scope.
• Collaborate with the team to meet departmental metrics.
